Forgot your Formitize Password?

  1. Open https://service.formitize.com.au/
    1. Click / Tap on "Forgot Password"
    2. .
  2. A new window will open.
  3. Enter your Company Name
    1. This is your Franchisee Code.
  4. Enter the Username.
    1. This is always Admin
  5. Submit
  6. The response should be... "Success - Your log-in details have been set to your registered email address"
